Harry Stack Sullivan fu tra i primi psichiatri a riflettere creativamente sull'universalità, nella specie umana, della tendenza al narcisismo, che considerò la madre di tutte le altre illusioni. Una tendenza ad agire e pensare come se, in fondo, la propria personale realtà (non quella altrui) non fosse toccata dalla morte, da molteplici vulnerabilità, dalla fallibilità e dall'assenza di amore. La tesi di Sullivan sulla universalità di una sorta di illusione narcisistica influenzò, probabilmente, altri Autori, fra cui primariamente Heinz Kohut, nella loro riflessione sull'esistenza di un narcisismo sano come ingrediente indispensabile per un positivo sviluppo della personalità. A partire dal ricordo delle idee di Sullivan, ci siamo posti alcune domande all'interno di una cornice intellettuale cognitiva ed evoluzionista: Qual è la differenza tra narcisismo sano e patologico? Cosa può indirizzare in senso patologico l'illusione narcisistica? Quali processi evoluzionistici conducono all'illusione narcisistica? Cosa hanno in comune le diversissime forme cliniche che può assumere il narcisismo patologico? Che implicazioni ha tutto ciò per la terapia? Questo articolo si sofferma sulle risposte che, in una chiave cognitivista ed evoluzionista, abbiamo tentato di dare a tali domande.
