I pazienti cronicamente distanti ed emotivamente isolati presentano spesso disturbi di identità. L'identità si sviluppa come modello tematico del narcisismo plasmato dalla natura delle influenze libidiche materne primarie sul senso di sé del bambino. L'identità fornisce una forma di auto-definizione che riguarda la domanda: Chi sono? Nel trattamento di questi pazienti, le resistenze alle vulnerabilità narcisistiche (le resistenze narcisistiche) procurano un senso di sicurezza illusorio e inducono l'analista a distogliere l'attenzione dal problema patologico centrale: il bisogno primitivo e terrificante de, le fantasie inconsce su, la dipendenza e funzionalità dall'altro. Il fatto che i pazienti sfuggano il materiale e le interazioni terapeutiche legate alle loro dipendenze è un aspetto di un tacito contratto con l'analista volto a impedire l'esame dei loro consistenti problemi di stabilità interiore. Tra questi problemi vi sono le angosce concernenti l'intrusione e la perdita della separatezza. Col procedere dell'analisi, gli elementi dell'identità di questi pazienti vengono chiariti ed usati per comprendere e organizzare il materiale, a vantaggio sia dell'analista che del paziente. Ciò permette al paziente di articolare un'esperienza di individualità più concreta e vitale. La presentazione di un caso clinico serve ad illustrare l'analisi di una paziente mediante l'uso di questo approccio.
