Il sistema immunitario inizia la sua ontogenesi in utero dove, per le fisiologiche necessità della gravidanza, si sviluppa secondo un profilo squilibrato in senso Th2, utile al corretto compimento della medesima e ad una nascita a termine. Una serie di fattori - relativi alla gravidanza, al tempo di gestazione, alle modalità del parto, al tipo di allattamento, al legame di attaccamento che si costruisce tra madre e bambino, all'esposizione a inquinanti, farmaci e ad alcuni vaccini, tramite meccanismi epigenetici - condiziona lo sviluppo del sistema immunitario, che raggiungerà caratteristiche simile a quelle dell'adulto solo alla fine dell'adolescenza. Il corretto funzionamento del sistema immunitario da adulto è largamente dipendente dall'assetto che esso acquisisce nelle prime fasi della vita.
